Is it safe to do quadrilateral (unilaterally like Siegfried) with small lengths (femurs = 4cm, tibias = 3cm)? Or is it safer to do all in the femurs (7cm)?

I know even Paley offers quadrilateral but is it really safe? Is there any research on stretching a total of 2mm per leg per day?

Some say, the early lengthening process is quite easy because the tissues can handle the stretch. But as you hit greater lengths, it becomes more painful. Does this hold true for quadrilateral lengthening also?

It's a safe length individually but if you combine them it's probably a riskier surgery even if you lengthen a little. It's like saying " Does getting punched hard in the stomach vs getting punched in the stomach/face hurt more ".

Besides I don't think I have ever talked to any quad patients that  lengthened so little ( most I talk to lenghten at least 10cm ). The amount of money spent on the procedure is probably the real long term problem unless you are rich.

The point is quite simple. The body is known to handle 1mm of soft tissue stretch per day. Can it handle 2mm of tissue stretch per day? If you lengthen just the femurs 2mm a day, you will develop nerve problems. But if you lengthen the femur 1mm and same side tibia 1mm will you not develop nerve problems?

Please leave cost and money aside from this thread. That is a personal matter and not a medical topic.

Would be way safer on so many levels to just do 7 CM on femurs and lengthen around 0.75-1 mm per day rather than break 6 bones and lengthen at 1.5-2 mm per day. Plus your body would heal much better, because you are way more mobile doing only one segment.

It also seems like many quadrilateral lengtheners start getting nerve problems relatively early in the process. There are some exceptions like Siegfried, but it seems like you are elevating the risk of something going wrong.

Thanks for the answer. I think I agree with you. Some people however say that the first 3-4cm on a segment are very easy and safe to achieve. Assayag said that 4+3 is better than 7 in the femurs. However as you said we don't know if that is offset by the increased rate of lengthening. Nerve problems are what I am concerned with the most in this approach too.
Can you ask Assayag when you meet him if you have time after your follow-up hahaha?

4cm+3cm is pretty safe on par with jogging at home, no matter what method you are using.
