Need help with Dr Guichet form pdf

I am trying to do this PDF form for Guichet.

I must confess I am not the best with computers for someone my age :/

For some of the longer form questions when I start writing my answer it just carries on in one line moving what I've written previously out of sight and to the left. I want it to start a new line when I carry on writing.

I am also not sure about how to attach a passport scan to the PDF.

I was trying to upload the file to the forum, it won't let me attach the file and says uploader is full.

You can convert pdf form to ms word doc and then insert the pictures to word. Also, There are softwares (http://www.pdfmerge.com/) to combine pdf's. So you can save your images as pdf and use the pdf merge tool to combine them with the form.
