do hospitals generally allow installment plan, for like 12~24 months?
how about HSS and Paley's?

You don't pay the hospital.  You pay the doctor and he pays for everything else.

That's true for Paley and probably all US doctors other than Rozbruch. You pay him and the hospital (HSS) separately.
