Would insurance companies cover this if you have leg discrepancy and wanted to add a few extra cm's? Or would you need to cover the 'cosmetic' bit yourself which probably includes extra time in the hospital and you needing surgery on the other leg.

that really depends on your insurance. leg descrepancy should be fixed due to long term effects on your whole body.
you need to talk to your insurance.

You will have to pay for everything except fixing the discrepancy.  No insurance company would pay for CLL, probably not even for dwarfism.
