hey guys,

I'm from Brazil. I've done tibias here by LON, I gained 4cm. I went from 152 to 156 lol. Now I intend to do femurs. The problem is that here in Brazil there is only LON or ISKD for Femurs. The FITBONE Nail is expected to arrive in the second half of 2023. But I can't wait until then and even then the surgery with her should be around 60k USD which is basically the price of a surgery with Dr GIOTIKAS with G - Nail. So I wanted your opinion:
Do I do ISKD here in my country or do I go to Athens and do G-Nail?

Things to Favor Here in Brazil:
ISKD: Much cheaper than G-Nail
The doctor is used to this rod, if there is a problem he has the solution to solve it, although in the last surgeries all of them had some kind of problem (Triggering the stretching, not wanting to stretch, etc)
I will be close to home and communication will be perfect.

Cons:
Fear of the ISKD Rod giving a problem
Does not support weight (I will have to be in a wheelchair)


G-NAIL

In Favor:

support my weight
more independence
treatment time should be shorter

Against:
More expensive than ISKD
I will be in another country (Language Barrier)

It really depends on how much money you have.  Definitely go abroad if you can afford it.

My vote is guichet-nail (gnail) with Dr. Giotikas (there are two Dr. G's).

I think Betz in Germany or Guichet in France are also good options and will be more luxurious experiences overall if you can afford them.

and my reason is:

- LON scars are worse than gnail scars I believe

Also this Dr. Betz interview where he talks about how ISKD is difficult to be done safely and also he talks about the problems of his older device fitbone:

A Spanish speaking patient in Athens bought some kind of translation device to help him communicate with everyone

Alternatively, grab a really small/light laptop and just type into google translate, I did it with some nurses whose English wasn't that great, it worked fine

Oh definitely go for gnail. Iskd and Fitbone is like getting a 90s flip telephone instead of a smartphone.

The biggest problem with iskd isn't that it can't support weight, it's the "run away nail".
In other words, the intramedullary nail may elongate without the patient's intention.
This can lead to non-union and neuropathy.

Fitbone in reality is much more sophisticated, like all magnetic nails, compared to obsolete nails like g nail or iskd.
However fitbone is completely unreliable and so is iskd. G nail and all the albizzia nails have only one benefit which is very significant though, full weight bearing ability. Other than that they are obsolete like all the mechanical ones which make lengthening phase a torture.

But compared to these two I would definitely choose g nail although I believe that precise 2 is the best nail in the present although it is not.full weight bearing.
