I'm pretty sure I'd like to start with tibias until safe limit and then lengthen femurs accordingly - Are there any doctors in the US who would be fond of hat idea? I know Paley is allegedly the best but this man doesn't like lengthening tibias first does he?

You don't have to tell them long-term plans. Just go there and ask for tibia lengthening.

Paley is totally open to whatever you want if you’re realistic, got the money, and in good mental and physical health. Go get a consultation with him, him and his staff are wonderful (I am not going with him for other reasons i.e. location/money/ability to lengthen from home, but I thought his center was perfect). They’ll do a fancy 3D x Ray and give you a recommendation based on your goals and proportions.
Personally unless you already have long femurs, I don’t get why you wouldn’t do femurs first, but that’s up to you.
