It's pretty much an opinion but i've read/heard enough anecdotal scenarios in which men of all heights chop on about 2-3 inches to their actual height. Especially under 6ft. When you realize that true 5'9 is much taller than thought of because so many 5'6-5'7 men are claiming to be and it is believable. Ever seen pics of guys in photos on their tip toes? It's humiliating but understandable.

P.S : Can we all boycott paley? I think he has a wretched character and is money hungry to maintain his Palm Beach social life persona but the fact of the matter is he is "new money" and as such I think helping those with a debilitating issue such as height neurosis would insight empathy and making individuals pay 125k+ at minimum is a sick and disturbing feat. It's hard not to feel incredible distain from a man that makes a living off our mental health. DISCLAIMER : nuvasive charges doctors so much less per nail than he is claiming. His secretaries and him are LYING about the cost and if you actually think this man isn't making profit (and good profit) off of us is naive. I say we boycott him and hopefully force his heavy cost down.

2"-3" is rare and most people inflating so much will be very likely be called out on it. Average heightfrauding is 1.5 inches at most.
Even then so from my viewpoint 5'9 doesn't look above average (I like to think I'm very good at guessing height) so regardless of what people claim 5'8-5'11 generally looks average range – even if the "perceived average" is 6ft or something.

Being a doctor is not charity you pay for what you get and for his top notch treatments and hardwork the cost is justified. And he does deformity correction more so it's not like the main motive is encashing your neurosis. So what if he profits anyway?

Unfortunately, the U.S. medical system is full of stuff like this.  Patients being overcharged for everything; hospitals and doctors even charging different patients of theirs different prices for the same procedure or product.  It's not unique to Paley; a lot of top doctors justify it as being commensurate with their skill.
