Just in theory, I know it costs 20k USD for removal but Im just wonder what if someone ran into a financial situtation where they couldnt afford the removal.

It gets more difficult to remove over time. The screws fuse with your bone. While you have the nails in you, you cannot take MRIs, which may be problematic if you need one in an emergency (trauma, cancer diagnoses, etc). There aren't enough long term studies that test the effects of nails inside bodies.
