Just hypothetically, I am aware that the cost for removal is $20,000, but I am simply curious about the scenario where an individual encounters a financial situation preventing them from being able to pay for the removal.

It becomes increasingly challenging to eliminate over time. The screws bond with your bone. While you have the nails in you, MRIs cannot be performed, which could pose issues in case of an emergency (such as trauma or cancer diagnoses). There are not adequate long-term studies assessing the impacts of nails within the body.
