Can someone add 3 inches and 4 inches to my femur? I just want to see if it will look weird

Based on that picture, I'd say your tibias are the part of your anatomy that are disproportionately short.  Maybe your endocrinologist just said to lengthen femurs because internal is the current standard of care, and internal femurs are less risky than internal tibias.

First off I can't do it so yeah....

Second that's not really the best picture to do so. The camera needs to be at waist level so both the legs and the torso are to some degree in equilibrium. In your picture it looks like you have long(ish) legs already.

In my humble opinion, all of them look "ok", but 3 inches femur looks most natural.

Based on that pic and the one MDOW posted, you should absolutely do tibias.
