I heard Janet Conway preemptively does IT band release and nerve decompression to prevent problems in the future. And I think Dr. Assayag does this too.

1. I thought to myself- well fk, it makes sense to me to loosen things up off the bat during the first surgery, should the patient ask to pay for IT band release/gastro release/nerve decompression upfront to be done during the surgery?

2. Are there other extra surgical moves the patient should request and pay for up front?

3. For doctors like Parihar and Giotikas, how much extra does it cost to request this and is it a bad idea to pressure them to do it during the first surgery?

Some doctors charge extra on top - on Debiparshad's website it lists the IT band release for example as an extra cost in addition to the actual surgery. Most others, those releases are included in the price.

During my consult with Dr Giotikas he said that an IT band release is routinely performed by him for all femur lengthening's and I believe it is the same with other notable docs like Paley, Rozbruch, Mahboubian, Lee etc.

In terms of gastro release / nerve decompression I believe they perform that on a case by case basis but I am not 100% sure.

Ah.... so basically Giotikas patients are not told whether there will be nerve decompression and he decides on the operating table?

I believe so. I am sure if you email them with those questions they would be able to give you a proper answer as they are usually quite responsive.

Dr Parihar does IT band releases for his internal femur lengthenings and there's no extra charge for it (Penguinn wasn't charged extra).

Thanks!!! What about nerve decompression and gastro release? I heard Janet Conway does it and I'm afraid of nerve damage (either from not doing or from doing it I'm not sure which is more dangerous)

Dr Parihar said in his interview that a gastrocnemius soleus recession is done if he determines there's starting tightness and that the patient would benefit from one. He included other necessary measures (which could include nerve decompression) as something that is determined on a case by case basis.

Oh hmm... so basically I'm trying to figure out if I need to budget in more money to pay for nerve decompression or releases upfront and how much they cost for different doctors. But it sounds like Giotikas and Parihar both don't charge extra for it?

I will also try calling Giotikas's office again, I was up at an odd time and managed to call when the Greece office was open despite the time difference and they said they'd email.
