Testing for HIV before surgery has nothing to do with getting it through blood transfusion. What is the relation? Blood is sourced from the government or other third party blood banks which is poorly regulated in India. If they do cheap testing which does not detect presence of HIV early on (some tests give correct results days after infection but some do only after weeks, so there is a window when the results are wrong), then they might have infected blood.
And general infection rate would probably be higher in India because it's more unhygienic but I don't have data to back this up.
